What You’ll Need when Applying

Here’s a quick list of what you’ll need (scroll down for more detail):

  • Gov’t-issued Photo ID

  • Proof of U.S. Citizenship or Residency

  • Proof of Nursing School Attendance/Acceptance

  • FAFSA Submission Summary

  • Brief Video Submission

You’ll also need the name and email address of the person(s) you will ask to write a Letter of Recommendation.

Gov't-issued Photo ID

You will upload an unexpired copy of one of the following:

  1. Driver's License, Learner's Permit, or MVA/DMV-issued Identification Card;

  2. Unexpired US Passport or unexpired Green Card; OR,

  3. US Military Identification Card.

Proof of U.S. Citizenship or Residency

You’ll upload an unexpired copy of one of the following:

  1. Unexpired US Passport or unexpired Green Card;

  2. Birth certificate showing birth in the United States;

  3. Form N-550, Certificate of Naturalization;

  4. Form N-560, Certificate of Citizenship;

  5. Form FS-240, Report of Birth Abroad of United States Citizen​; OR,

  6. Green Card or other document showing legal residency status in the U.S.

Proof of Nursing School Attendance/Acceptance

You’ll upload one of the following:

  1. If you ARE currently attending nursing school, you will upload an UNOFFICIAL cumulative school transcript which identifies you as the student, indicates coursework consistent with your pursuit of a certificate or degree in the field of nursing; and, contains a full history of courses taken, grades earned, and includes a cumulative GPA.

  2. If you ARE NOT currently attending nursing school, you will upload the letter, email, or document you received stating that you have been accepted into nursing school for the coming fall semester (or earlier if applicable).

FAFSA Submission Summary

You’ll upload all pages of your FAFSA Submission Summary for the coming semester.

Brief Video Submission

During the application you’ll upload a brief selfie video. We recommend recording your selfie video before you start the application to help ensure you complete the application in one session. Before beginning the application, you might want to ensure you are someplace with good lighting that’s quiet enough to allow you to record the video uninterrupted.

  • Your video should be no more than 5 minutes long.

  • Some topics to cover in your video:

    • your background;

    • why you are interested in nursing;

    • where you are in your nursing journey (your nursing experience and education);

    • your plans for the next 10 years; and/or,

    • anything else you want us to know about you and your application.

​Letter of Recommendation

(Note: you’ll send a request for Recommendation during the application. Please read below, then think about the name and email address of the person to whom you will send the request for recommendation.)

If you have CURRENT or PAST EMPLOYMENT IN NURSING: a Letter of Recommendation from your supervisor or member of management is required; OR,

​If you are/were a student: a Letter of Recommendation from one of your teachers (NOT an advisor or counselor) is required.

Only one Letter of Recommendation is required; however, you may opt to request a second Recommendation if you think that will help your application for scholarship. Again, the second recommendation is NOT required, nor will it guarantee a higher likelihood of scholarship award.

Be sure you have the documents and information you’ll need before beginning the application. Having these before you start will allow you to complete the application in one session of about 10 minutes.

When you have everything and are ready to begin the application, click the Apply Now button below (this will open a new window and send you over to our secure application partner, Submittable). Then:

  • click the Sign Up link;

  • create a username and password; and,

  • complete your application and upload some required documents.
